Certificate Requests

REQUEST

You must come IN PERSON to the Main Parish Office (inside Main Church building) and fill out an Official Request Form.

The Main Parish Office hours are Monday through Friday, 9:00am to 12:00pm & 1:00pm to 5:00pm.

*Only the actual recipient and/or the legal guardians may request official certificates. We may request to check a picture I.D.

For individuals requesting official certificates for:

• Baptism
• Confirmation
• First Holy Communion
• Holy Matrimony (Marriage)

PICK-UP

You will be called when the certificate is ready for pick up.
